For that LO, the frame pretty much already looked like that. You can get it for free here http://www.digitalscrapbookplace.com/gallery/showphoto.php?photo=115140&cat=754 You will need to register and log in to download but is a freebie there. What I did to make it look like it was laying a little bit flatter was to change the size on it but I only adjusted the height and left the width the same.
For adding the photo in there and giving it that effect, what I did was pretty easy. I placed the photo underneath the frame layer and the duplicated it and made it invisible to come back to in a minute. I cut off the edges of the photo that were showing around the frame. Then I made the duplicate layer visible again and changed the layer order to put it on top of the frame and cut around my baby and her ball. I added an overlay to the photo frame to grunge it up a little and added a drop shadow. It was actually pretty easy, I hope this helps and if you need more info, just let me know ok? :)
